Q. What are the Qualifying Conditions for MMJ

  1. Cancer
  2. Glaucoma
  3. Positive status for human immunodeficiency virus/ acquired immune deficiency syndrome
  4. Hepatitis C
  5. Amyotrophic lateral sclerosis
  6. Tourette’s syndrome
  7. Crohn’s disease
  8. Ulcerative colitis
  9. Post-traumatic stress disorder
  10. Severe arthritis
  11. Fibromyalgia
  12. Alzheimer’s disease
  13. Cachexia or wasting syndrome
  14. Peripheral neuropathy
  15. Intractable pain which is pain that has not responded to ordinary medications, treatment or surgical measures for more than six (6) months
  16. Severe nausea
  17. Seizures including without limitation those characteristic of epilepsy
  18. Severe and persistent muscle spasms including without limitation those characteristic of multiple sclerosis
  19. Any other medical condition or its treatment approved by the Department of Health

Q. How do I apply for the Medical Marijuana Program?

Applications may be completed online at: http://www.healthy.arkansas.gov/programs-services/topics/id-card-apply-online

The medical marijuana application process has been made easy as possible for individuals to complete. The application lists the requirements. Read the application carefully.

Qualifying patients may submit applications using the Department’s website or applications may be printed from the website and mailed in to the department. If requested, the department will mail or email an application to a patient.

Applications are generally processed in the order in which they are received.

Q. Is my confidentiality protected when I apply and if I am approved for use of medical marijuana?

Yes. The following information received and records kept by the Arkansas Department Health Medical Marijuana Section are subject to all applicable federal privacy laws, are confidential, are exempt from the Freedom of Information Act and are not subject to disclosure to any individual or public or private entity, except as necessary for authorized employees of the department to perform official duties for the medical marijuana program. Law enforcement will have the ability to confirm patient status in the medical marijuana registry only if a patient is approached or arrested by a state or local law enforcement officer.

Applications, or renewals, their contents and supporting information submitted by qualifying patients and designated caregivers, including information regarding designated caregivers and physicians

The individual name and other information identifying the person to whom the Arkansas Department of Health Medical Marijuana Section has issued registry identification cards

Q. What information and forms are required when a patient applies for a Medical Marijuana ID card?

The following are required when submitting your application:

  1. The application form.
  2. The completed Physician Written Certification.
  3. Photocopy of your Arkansas issued driver's license or state ID (remember: name and address must match what is listed on your Arkansas driver's license or ID).
  4. The nonrefundable $50 State of Arkansas application fee. (Physician Certification Fee is Separate)

Q. Will the Arkansas Department of Health verify my written certification form with my doctor?

Yes. The department may contact the physician that provided the written certification.

Q. How long does it take to process an application once I submit my forms to the program?

When a fully completed application has been received, the department has 14 days to process the application.

Once processing is complete, the applicant will receive notification. MMJ cards will not be issued until one month prior to Medical Marijuana availability in Arkansas dispensaries.
